Documents to Bring to Your Appointment
Important: You must provide your photo ID and late notice to be seen.
- Government Issued Photo ID for all adults in the household
- Social Security Cards for all household members (government issued documents showing the full 9-digit SSNs will be accepted)
- Late Notice for the rent, mortgage, and/or utility bill for which you are requesting assistance
- Rental or Lease Agreement (if expired, provide most current one)
- Proof of Income for the last 60 days from all sources for all household members
- Proof of Basic Expenses for the last 30 days

- Benefits Information such as food stamps letter or child support verification letter (if applicable)
- Proof of Crisis Situation to include verifiable documentation of a decrease in income or increase in basic expenses
- Bank Statements to include previous 60 days of bank/pay card transaction history.
