Gatekeeper Goal: Health Assessment on myCigna
The Health Assessment on myCigna is required for employees to unlock all their myOCWellness incentive points.
Watch the video below to find out how to locate and complete the Health Assessment on myCigna.
It is highly encouraged for spouses to complete the Health Assessment, but this is not a requirement for the
spouse.
You can also follow these steps to complete the Health Assessment:
- Log-in to myCigna
- Hover over the Wellness tab
- Click Health Assessment.
- Select Take My Assessment.
- Read the Welcome screen and select Next to begin.
- Read and select I Agree to the Privacy Statement. Note: You must select I
Agree to complete the assessment.
- Select the answers to your Gender, Race, and Date of
Birth
and hit Next.
- Complete all 5 categories by answering all the questions. You will receive a
Congratulations screen indicating the completion of each category.

You may Save and Exit the Health Assessment at any time but be aware that you may need to
repeat incomplete categories.
- The Health Assessment will time out after 15 minutes of inactivity.
- If you Save and Exit the Health Assessment, your progress will be saved for 7 days.
If you access the Health Assessment again after the 7 days, you will need to start over.
- Once you have completed the Health Assessment, you will receive a score. This score indicates that the
Health Assessment has been marked as complete, it does not have any effect on your incentives. The
assessment is a tool to gauge just for you and shares the information on what you need to work on in your
personal health and wellness. The results of your health assessment are confidential and will not be shared
with anyone.
- The Health Assessment can be completed each year, and at any time during your incentive period. Also,
completing this goal earns you
20 myOCWellness points!
Self-Report Points
To self-report your points, log into mycigna.com and follow
the steps below:
- Click on "Wellness" at the top of your screen
- Scroll down and click on "Wellness and Incentives"
- Click on “View all Incentives”
- Scroll down and click on “Self Reported Activities”
- Scroll down to the item you are reporting
- Input the date you completed the goal and click on “Complete Goal”
The following activities must be self-reported in mycigna.com
- Participate in Countywide Wellness Events (Wellness Expo, Health Summits, Financial Wellness Expo)
- Participate in IOA Corp 5K or other community organized event 5K or greater
- Participate in Wellable Challenge(s)
- Complete OCG Life Balance Series myOCLearn
- Get the COVID-19 Vaccine
- Get an annual vision/eye exam (with an optometrist/ophthalmologist)
- Participate in County Sponsored Wellness Webinars
- Get a shingles vaccine
- Get a pneumonia vaccine
- Participate in blood donation
- Participate in using Headspace, the mental well-being application
- Participate in getting your blood pressure screening with onsite Wellness Nurse
