Health Savings Account (HSA):

You are required to have an open and active health savings account with Cigna/ HSA Bank.

You must apply for your HSA account through the Cigna portal no later than November 2, 2018.

HSA: For 2019, the Federally mandated increase in the HSA contribution limits are $3,500 for individuals and $7,000 for families. Catch-up contributions (age 55 and older) are $1,000.

MEDICAL UNDERWRITING RULES ARE IN EFFECT FOR THE 2019 OPEN ENROLLMENT PERIOD.

Additional Employee Life – Requests to elect or increase coverage by $10,000 , up to the guaranteed limit of $200,000 will not require medical underwriting unless you have been denied in the past. Elections or increases above the guaranteed limit of $200,000 will require medical underwriting.

Spouse Life – Requests to elect or increase coverage by $10,000 up to the guaranteed limit of $50,000 will not require medical underwriting unless your spouse has been denied in the past. Elections or increases above the guaranteed limit of $50,000 will require medical underwriting. Remember, spouse life cannot exceed employee basic life plus employee additional life.

Child Life – Requests to elect or increase Child Life Insurance, will not require medical underwriting.

Short-Term Disability – Requests to elect or increase your STD waiting period by one step will not require medical underwriting unless you have been denied in the past. Any request to elect or reduce your STD waiting period greater than one step will require medical underwriting.

Don’t forget to submit your medical underwriting form to The Standard by November 2, 2018, if applicable.

OrangePrime

This plan is the Cigna Low Deductible Health Plan (LDHP) with $750 / $1,500 deductible and 20% co-insurance once the deductible is met. Your out-of-pocket maximum is $2,100 / $4,200 depending on your coverage. Co-payments apply to office visits, urgent care and prescription drugs. Your prescriptions are covered $10 (Generic), 10% + $30 (Preferred), 10% + $50 (Non-Preferred).

OrangePrime Plus

This plan is the Cigna High Deductible Health Plan (HDHP) with $1,500 / $3,000 deductible and 20% co-insurance once the deductible is met. Your out-of-pocket maximum is $3,000 / $6,000 depending on your coverage level. Your preventive prescriptions are covered: $10(Generic); 10%+$30 (Preferred); 10%+$50 (Non-Preferred).

Humana Vision Plan

The Plan is offered by Humana CompBenefits Vision Care Plan. The plan covers routine eye examinations, corrective lenses every 12 months with co-pays. Frames are covered every 24 month with co-pays. Discounts for laser vision correction are available through TLC Vision Advantage.
